The process usually involves a complete physical exam, a review of the applicant’s medical history and immunization records, as well as laboratory tests to check for any infectious diseases.

Obtaining a Permanent Resident Card (Green Card) in the United States can be an exciting experience, however, everyone applying for a Green Card needs to take part in the immigration physical exam, also known as the green card medical examination. This examination is an important step in the immigration process and can only be conducted by a designated Civil Surgeon, who is a physician authorized by the U.S. Citizenship and Immigration Services (USCIS).

The main purpose of the immigration physical exam is to determine the medical fitness and health of the applicants and to ensure that they have all the necessary vaccines as per the guidelines of the USCIS.

The results of the green card medical exam are then used to determine the eligibility of the applicants for their permanent resident status in the United States.

Immigration Physical Exam Requirements

Adult Exam Requirements

  • A Physical Examination
  • QuantiFERON tests (blood draw)
  • If the test result shows positive for TB, chest X-ray is required.
  • If the chest X-ray results show active TB lesion(s) or suspicious finding(s), the applicant is mandatorily referred to the county health department for further evaluation/treatment.
  • Syphilis (blood draw)
  • Gonorrhea (urine test)
  • A review of vaccination records. Immunizations are dependent on age and medical history.

Adult Immunization Requirements

  • Measles, Mumps, Rubella Vaccine (MMR)
  • Tetanus/Diptheria Vaccine (Tdap)
  • Varicella (Chicken Pox) Vaccine
  • If you have had a history of varicella, vaccination can be waived but required to test immunity by blood test.
  • Hepatitis B
  • 2 doses for Pfizer/Moderna Vaccines OR 1 dose of Janssen Vaccine
  • Influenza (Flu)
  • You will be required to have the seasonal flu vaccine if your immigration medical exam falls in the flu season (October 1 – March 31)

Kids Exam Requirements

  • A Physical Examination
  • QuantiFERON tests (blood draw) – 2 years & older
  • If the test result shows positive for TB, chest X-ray is required
  • If the chest X-ray results show active TB lesion(s) or suspicious finding(s), the applicant is mandatorily referred to the county health department for further evaluation/treatment
  • Syphilis (blood draw)
  • Gonorrhea (urine test)
  • A review and update of the pediatric vaccinations. Immunizations are dependent on age and medical history.

  • Rotavirus Vaccine
  • Measles, Mumps, Rubella Vaccine (MMR)
  • Tetanus, Diptheria, Pertussis Vaccine
  • Polio Vaccine
  • Haemophilus Influenzae Type B Vaccine
  • Hepatitis B Vaccine
  • Hepatitis A Vaccine
  • Pneumococcal Vaccine
  • COVID-19 Vaccine

The immigration physical exam can be a daunting experience, and it is important to be prepared to ensure a successful outcome. Having the right documents and being aware of what is required before and during the exam can help reduce stress and avoid delays or issues that can lead to a denial of the application.

We advise that you complete the application form I-693 (Part 1) before your appointment.
Do not sign or date the form until the doctor instructs you to do so.

At the end of the exam, the doctor will complete his/her portion of the form.

  • This includes the immigration medical exam, blood draw, required lab work, vaccination status verification, paperwork processing, and certified and sealed I-693 form.
  • All vaccine fees could be avoided by bringing vaccination records.
  • Vaccine Prices Vary. If you need additional vaccines, we will discuss how to receive the vaccines for the lowest cost possible.
Payment must be made in cash, debit or credit card only. We do NOT offer chest X-Ray.

What to Bring?

  • Valid passport or other government-issued photo identification such as state ID, driver’s license, travel permit, or work permit.
  • Alternative proof of identity may be accepted for children under 14 years of age that shows name, date and place of birth, parents’ full names and other identifying information.
  • Immunization / Vaccination records (English Translation), including proof of COVID-19 vaccination, unless it is not available for you (e.g if you are under 6 Month old).
  • Form I-693 (Fill Out Part 1 before coming in).
  • Alien Registration number if you have one (starts with an “A”).
  • The required fee for the exam – Cash, Debit and Credit Cards only.
  • An Interpreter who can speak, read and write in English if needed.
  • List of medications (if you are being treated for a chronic medical condition or taking medications on a regular basis) or a letter from your regular doctor outlining the treatment plan for any health problems you have.
  • Tuberculosis certificate from your doctor (if you’ve had a previous positive skin test for tuberculosis) proving that you were adequately treated.
  • Certificate of clearance signed by a doctor or public health official, proving that you were adequately treated (if you have had syphilis).
  • If you have a history of harmful or violent behavior resulting in injury to people or animals, information that will allow the doctor to determine whether the behavior was related to a psychiatric or medical problem, or to drug or alcohol use.
  • If you have been treated or hospitalized for psychiatric or mental illness, or alcohol or drug abuse, written certification including the diagnosis, length of treatment, and your prognosis.

After The Exam

After the examination is completed, we will complete, sign, and seal the I-693 Form and include any supporting documents in an envelope that would read DO NOT OPEN. You must submit the sealed envelope to USCIS as directed in the Form I-693 instructions.
