What does a Pap smear check for?

A Pap smear checks cervical cells for changes that may be normal, unclear, abnormal, or precancerous. It is mainly used for cervical cancer screening, not as a complete test for infections, ovarian cancer, uterine cancer, or all sexually transmitted infections.

Is a Pap smear the same as a pelvic exam?

No. A pelvic exam is a physical exam of the reproductive organs, while a Pap smear is a lab screening test that collects cells from the cervix. A Pap test may be done during a pelvic exam or well-woman visit when appropriate.

What is the difference between a Pap test and an HPV test?

A Pap test looks for abnormal cervical cell changes. An HPV test looks for high-risk types of human papillomavirus that can cause cervical cell changes and cervical cancer. Depending on your age and history, your provider may recommend a Pap test, HPV test, or both.

When should I start getting Pap smears?

Many patients begin cervical cancer screening at age 21. Your provider can confirm the right start time based on your age, health history, prior results, immune status, and whether you have a cervix.

How often do I need a Pap smear?

Screening frequency depends on your age, test type, results, and medical history. Many patients ages 21 to 29 are screened with a Pap test every 3 years if results are normal. Patients ages 30 to 65 may have options such as Pap testing, HPV testing, or co-testing at different intervals.

Do I need a Pap smear every year?

Not always. Many patients do not need a Pap smear every year if recent results were normal. However, you may still need a yearly wellness visit, birth control discussion, STI screening, breast exam, vaccine review, or other preventive care.

Do I still need Pap smears if I had the HPV vaccine?

Yes, many vaccinated patients still need routine cervical cancer screening. HPV vaccines reduce risk but do not replace Pap smears or HPV testing because they do not cover every possible cause of cervical cancer and may not protect against exposure before vaccination.

Do I need a Pap smear after a hysterectomy?

It depends on whether your cervix was removed and why the hysterectomy was done. Some patients no longer need routine cervical cancer screening after total hysterectomy for non-cancerous reasons, while others need continued follow-up because of prior abnormal results, cervical precancer, cancer, or other risk factors.

Can I stop Pap smears after age 65?

Some patients can stop cervical cancer screening after age 65 if they have had adequate normal prior screening and no history that increases risk. Your provider can review your past Pap, HPV, biopsy, and treatment history before deciding whether screening should continue.

HPV Testing & Cervical Cancer Prevention

What is HPV and how is it related to Pap smears?

HPV, or human papillomavirus, is a common virus. Some high-risk HPV types can cause cervical cell changes that may become cancer over time. Pap smears look for cell changes, while HPV testing checks for high-risk HPV types.

Can a Pap smear tell if I have HPV?

A Pap smear by itself looks at cervical cells. HPV testing is a separate lab test that may be ordered with the Pap sample or at a different time. Your provider can explain whether your screening includes HPV testing.

Does a Pap smear test for STIs?

A Pap smear is not a full STI test. If you want testing for infections such as chlamydia, gonorrhea, trichomonas, HIV, syphilis, hepatitis, or herpes concerns, ask your provider which STI tests are appropriate for you.

Can cervical cancer be prevented?

Cervical cancer risk can often be reduced through routine screening, timely follow-up of abnormal results, HPV vaccination when appropriate, safer sex practices, and avoiding smoking. Screening is important because early cervical changes usually do not cause symptoms.

Preparing for Your Pap Smear

How should I prepare for a Pap smear?

Before a Pap smear, it is often best to avoid intercourse, douching, vaginal medicines, and spermicidal products for about 2 days unless your provider gives different instructions. Please schedule your Pap smear when you are not actively bleeding. Bring your medication list, prior Pap or HPV results if you have them, and any symptoms or concerns you want to discuss.

Can I get a Pap smear while on my period?

Altheda generally recommends scheduling your Pap smear for a day when you are not actively bleeding. If your period starts before your appointment, it is usually best to reschedule unless your provider gives different instructions.

What happens during a Pap smear appointment?

During a Pap smear, your provider gently places a speculum in the vagina to see the cervix, then collects cells from the cervix with a small brush or swab. The sample is sent to a lab, and your provider will explain how results are shared and when follow-up may be needed.

Does a Pap smear hurt?

A Pap smear is usually quick, but some patients feel pressure, cramping, or brief discomfort from the speculum or sample collection. Tell your provider if you are nervous, have pain with exams, have a history of trauma, or need a slower, more comfortable approach.

Can I have sex after a Pap smear?

Many patients can return to usual activities after a Pap smear. Mild spotting or cramping can happen. If you have significant bleeding, pelvic pain, fever, or symptoms that concern you, contact a healthcare provider.

How long do Pap smear results take?

What does a normal Pap smear result mean?

A normal or negative Pap result means no abnormal cervical cell changes were found in that sample. It does not mean you never need screening again. Your provider will recommend when your next Pap smear or HPV test should be done.

What does an abnormal Pap smear mean?

An abnormal Pap smear means cervical cell changes were found. It does not automatically mean you have cancer. Many abnormal results are related to HPV, inflammation, infection, hormonal changes, or mild cell changes that need monitoring or follow-up.

What does an unclear or unsatisfactory Pap result mean?

An unclear result may mean the cells are not clearly normal or abnormal. An unsatisfactory result may mean the lab did not have enough readable cells, sometimes because of blood, mucus, or clumping. Your provider may recommend repeat testing or HPV testing depending on the result.

What happens if my HPV test is positive?

A positive high-risk HPV test means a type of HPV linked with cervical cancer risk was found. It does not mean you have cervical cancer. Follow-up may include repeat testing, Pap testing, genotyping, colposcopy, or monitoring based on your age and prior results.

What is a colposcopy?

A colposcopy is a follow-up exam that uses magnification to look closely at the cervix after certain abnormal Pap or HPV results. A small biopsy may be taken if an area looks concerning. Your provider can explain whether colposcopy is needed and help coordinate referral when appropriate.
